Healthy Connections is a program of Welcome to America, a community-based nonprofit 501(c)(3) organization (#5698276). Community health worker services described on this site are provided by that organization and its staff.

The content on this website is provided for general information about our community health worker services. It is not medical advice, legal advice, or a guarantee of eligibility for any health plan, benefit, or program. Eligibility for Medi-Cal and for specific health plans is determined by the State of California and the plans themselves.
No cost to members
Community health worker services are provided at no cost to eligible Medi-Cal members. We will never ask you to pay for these services. If someone requests payment on our behalf, please contact us at 833-249-1563.
Program availability
Specific programs, including gift card incentives, are subject to eligibility requirements, health plan participation, geographic availability, and funding. Program terms may change or end at any time.
